The Participant Release Form is required for all youth participants. This form asks for camper information, parent/guardian information, emergency contact details and signatures by camper and parent/guardian on our site wavier.

Adult Participant Release Form

Contact Information and Site Wavier

The Adult Participant Release Form is required for all adult participants. This form asks for participant information, emergency contact details and signatures by camper participant on our site wavier. We also request health history and dietary requirements, as adult participants are not required to fill out our Health Form. The only exception to this is for adult participants in our offsite high school programs.

Family Camp Participant Release Form

Every family camp member must complete their own participant release form. This form collects essential details, including participant information, health data, family member information, emergency contact information, and signatures.
***Please be aware that this form is specifically for FAMILY CAMP participants, including both youth and adults.***

Epi-Pen Care Plan Form

The Epi-Pen Care Plan is required solely for campers who need an Epi-Pen. This document is designed to detail the care plan for your camper in case their Epi-Pen needs to be used while at camp.

Asthma Care Plan Form

Our Asthma Care Plan is only necessary for campers who have asthma. If your child does not have this condition, this form is NOT needed.

The purpose of this form is to outline a care plan that we'll follow at camp should your camper experience an asthma attack or have difficulty breathing.

Health Form

Health History and Doctor's Release

All youth participants must submit a completed Sky Ranch Health Form. This form requires signatures from both a parent/guardian and a physician. It is crucial that these signatures are on this specific form, not on any other document! Furthermore, the doctor's visit should have occurred within the past 24 months. Camper immunization records are also necessary on this form.
